Applicants to the 2009 SHO matching scheme are reminded that the deadline for submission of Appraisal Forms is midday Wednesday 25th February 2009.
Applicants are responsible for ensuring that two Appraisal Forms are submitted to RCPI before the deadline. Applicants applying to more than one specialty must ask their supervising consultants to complete the relevant form for each specialty. Appraisal Forms may be faxed to 00353 (0)1 672 4706 or posted to RCPI, Frederick House, 19 South Frederick Street, Dublin 2.
At least one Appraisal Form must be from a supervising consultant worked with in the last 6 months. This can be a current supervising consultant OR a consultant worked with during the period July - December 2008. The second Appraisal Form can be from any other consultant worked with in a substantive (paid) capacity. Applicants cannot submit Appraisal Forms covering a period spent in a supernumerary post or clinical attachment.
