This is a retrospective record review study of adverse events in Irish hospitals. The study was carried out by the Royal College of Surgeons in Ireland (RCSI) in collaboration with RCPI. Six nurse reviewers, each with a minimum of seven years’ nursing experience and all having experience in clinical research, audit, hospital management and/or education and three physician reviewers (two recently retired respiratory physicians and one public health medicine physician) performed the chart reviews. The study was published in BMJ Quality & Safety on 9 February 2016.
